• Portal Access Request Form (PAR). The PAR form is used to gain access to DORIIS. Every individual who will be accessing DORIIS is required to complete a PAR form. (PDF, 177 KB)

• Delegation of Authority (DOA). The DOA form applies to recyclers only, and is used to authorize a receiver of material to enter information into DORIIS for a recycler. (PDF, 48 KB)

Important: If you are shipping to a receiver who is located in Deployment Area 1, you need to insure the Program has the appropriate paperwork on file for your site (PAR or DOA). Failure to complete the appropriate paperwork may hinder/delay your ability to ship material on or after March 16, 2009.
