New Closed Site Inspection Form (Form 188)

This all LEA e-mail was sent on behalf of the LEA Support Services Branch, and the Remediation, Closure and Technical Assistance Branch of the California Integrated Waste Management Board.

In response to both LEA and CIWMB staff requests and to make inspection record keeping more efficient, CIWMB staff has combined the Closed Site Inspection Report (Form 55) and the Closed Facility Inspection Report (Form 56) into the new Closed Disposal Site Inspection Report (Form 188) (Fillable PDF, 830 KB). Therefore, LEAs will no longer have to choose which form to use. This is especially beneficial at sites with both older and newer closed areas.

Effective August 1, 2004, Form 188 is to be used for ALL closed disposal sites. LEAs may begin using Form 188 now, but must begin using the form no later than August 1.  Please be aware that Form 188 is to be used once a site has completed closure activities (i.e., all CIA sites and those closure plan sites where closure construction is complete). The active Disposal Facility Inspection Report (Form 52) shall be used prior to completion of closure activities,  including during closure construction.

Attached is the new, combined Closed Disposal Site Inspection Form and instructions (Form 188) in Excel/Word and PDF formats. Please note the following:

  • The included state minimum standards (SMS) were slightly revised (i.e., 20919.5 was added, 21110 was deleted).
  • Form 188 is available on the CIWMB web site. Forms 55 and 56 will be removed from the web in the near future.
  • Three-part hard copies forms will be printed but will not be available until September 30, 2004

In furtherance of the CIWMB's waste reduction goal, we ask that LEAs inform Randy Hobson as to the number of printed forms that will be needed. We ask that LEAs only order the number of forms that will be needed between now and the end of the calendar year. As always, LEAs can use the attached files or download from the Web site in lieu of using the printed, 3-part forms. The Excel format is designed to be screen/keyboard enterable, while the PDF format is designed for printing and copying and hand entry.
