Waste Tire Program : Request for Comprehensive Trip Log Forms

Effective January 1, 2006, all used/waste tire haulers are required to use the Comprehensive Trip Log (CTL) form (CalRecycle 203). Please read about the CTL requirements and procedures before ordering.

As part of our service to waste tire haulers, the Department of Resources Recycling and Recovery (CalRecycle) provides online ordering for the CTL, using the form below. The forms are available in the following formats:

  • Individual CTL forms (up to 50)
  • Package of 100 CTL forms(maximum of 5)
  • Box of 700 CTL forms (maximum of 3)

Please note that you must be a registered waste tire hauler or authorized exempt hauler to request the CTL form and must enter your Tire Program Identification number for the ordering process to be completed. More information on obtaining a Tire Program ID is available.

To request forms by mail submit a completed CalRecycle 681: Comprehensive Trip Log and Receipts Order Form (MS Word, 40 KB | Adobe PDF, 521 KB) by mail or call toll free (866) 896-0600 (English and Spanish).
