Tire-Derived Product Survey

The California Department of Resources Recycling and Recovery would like to hear about your satisfaction level with the tire-derived product used in your grant project. Use the link below to access the online survey. If you are unable to access the online survey for any reason, you may download, print, and mail in the survey. The information you provide is very valuable to CalRecycle. Thank you in advance for your participation.

Before taking the survey, there are a few points to keep in mind:

• Allow 10 minutes to take the survey.
• Do not close your Internet browser while taking the survey as all information will be lost.
• Have your grant number available.
• If applicable, please gather any reports or statistical information/results regarding the tire-derived product to include in the appropriate section(s).
